A user (noneditor) reports that after adding a couple pages to a category, they were put there correctly. However, after another user flagged these versions as sighted, they were removed from the category page. This can be seen in these images: [[de:Dorfkirche Friedersdorf]] is in the category [[de:Kategorie:Baudenkmal_im_Landkreis_Märkisch-Oderland]]: http://de.wikipedia.org/wiki/Datei:Bugdorfkirchefriedersdorf.PNG However, the page is not listed in that category, even though it was there before: http://de.wikipedia.org/wiki/Datei:Bugkategoriekirche.PNG. Subsequent edits solve that problem, purging does not.
Is this reproducible?

Yes, see the screenshot http://de.wikipedia.org/wiki/Datei:Bugdorfkirchefriedersdorf.PNG. As for reproducability, I do not know, but the user claims that this has occured several times. It definitely does not occur always.
I just observed something similar. [[de:Flatterball]] had [[de:Vorlage:Löschantragstext]] transcluded and list=embeddedin listed the article as transcluding the template; an hour later another user sighted the revision and then neither the API nor the corresponding special page listed the transclusion. When some minutes ago another user edited the article again (and automatically sighted it) the listing reappeared.
