I have installed  mediawiki(1.17Beta)_zh and extensions:FCKeditor(official) in the share hosting. I could not see the FCKEditor toolbar and not show [Rich Editor] [Open Rich editor in new window]. Just change the content:
from :[[MediaWiki:Edittools]]
to :</li><li><a _fcknotitle="true" href="MediaWiki:Edittools">MediaWiki:Edittools</a>
I have the following options set in the  /extensions/FCKeditor/FCKeditor.php

$wgDefaultUserOptions['riched_use_toggle'] = 1;
$wgDefaultUserOptions['riched_start_disabled'] = 1;
$wgDefaultUserOptions['riched_use_popup'] = 1;
$wgDefaultUserOptions['riched_toggle_remember_state'] = 1
