What do you even mean? All permissions as in copyright? Or as in userrights? What would this flag do?

As in userrights. The phrase "all rights reserved" is used here as a short, humorous alternative to "all rights/permissions cannot be revoked". That flag prevents userrights revocation of regular bureaucrats. Only special bureaucrats (one more userright) can do so.

That means protect all userrights of a user. Once flagged with this flag, his/her also immune from blocks. You can protect some (not all) userrights and/or prevent blocks.

Looks too exotic a feature for MediaWiki core - probably, it would be more reasonable to implement it as extension?

You should create an extension called Extension:AllRightsReserved.
