I suggest to add <label>s for the lock and unlock confirmation forms. 

Currently, the page source is

<td style="text-align:right"><input type="checkbox" name="wpLockConfirm" /></td>
<td style="text-align:left">Yes, I really want to lock the database.</td>

Please add <label>
