(logged-off only in Opera, Firefox and IE ;)) at apparently random occurence a wrong version of flaggedrevs.js is loaded which is a 404 error subsequently leading to a JS error: 

http://bits.wikimedia.org/w/extensions-1.17/FlaggedRevs/client/flaggedrevs.js?87&301-3 

in a next try (random article function) it is: http://bits.wikimedia.org/w/extensions-1.17/FlaggedRevs/client/flaggedrevs.js?87&301-4 which is okay (304)

The same happened on 2011-10-01 https://bugzilla.wikimedia.org/show_bug.cgi?id=31301

Sounds like those are on cached pages rendered by MediaWiki 1.17 and still sitting in proxy caches.

Looks like that issue fixed itself probably by automatic cache expiration. So I am assuming it is fixed.
