http://commons.wikimedia.org/wiki/File:Toll_Texas_1.svg The first (oldest) version doesn't render if it is the current version - but it does if it is just a archive version with another version being the current version.
Seems ok reuploaded here: https://test.wikipedia.org/wiki/File:Toll-test.svg
You did upload another version (whichever) - yes it did render at testwiki. But: I have uploaded the first (oldest) version now and it doesn't render. Like in Commons. Different to Commons is: The old version renders if it is not the current version: https://upload.wikimedia.org/wikipedia/commons/thumb/archive/a/a2/20111014162523!Toll_Texas_1.svg/127px-Toll_Texas_1.svg.png
I could have sworn i just made a post here. Basically, its due to our svg reader not substituting entity references in the xmlns attribute (probably upstream issue with the xml lib we use). svg reader can't find width and height, takes it to be 0, lack of rendering occours.

I would guess same bug combined with caching the image metadata so the oldest thumb isn't broken yet.
This is now fixed. All the caches have went away
