tested with FF 7.0.1 on Linux, worked. http://test.wikipedia.org/wiki/File:Paper_to_delete.jpg http://test.wikipedia.org/wiki/File:Occupy_NOLA.jpg Although, Hrm, that does show Template:Udm instead of the Ukranian template
offhand this is more likely to do with tbe form data flow of adding multiple languages than the script used.
Seems to work ok with Firefox 3.6.24 on Mac OS X 10.7 as well: http://test.wikipedia.org/wiki/File:Some_fake_title_testing.png There's not a local 'Template:uk' on test which is why the second descr doesn't show there, but it's in and the upload process seemed to go fine. No other differences visible uploading on commons vs test; english/ukranian or russian/ukranian description pairs go through fine for me.
I (FF 3.6, Linux) tested it on https://test.wikipedia.org/wiki/Special:UploadWizard with interface language German and it worked. Last step displays: [[File:Burning eyes smileyig.png|thumb|Bildbeschriftung hier hinzufügen]] https://test.wikipedia.org/wiki/File:Burning_eyes_smileyig.png
