The Special:UnusedFiles page lists used files that are embedded with "Image:" rather than "File:" in the wikicode (i.e. "[[Image:Kumquats.jpg]]"). This seems to cause it to ignore usage of the file even though it's successfully embedded in the page. This was puzzling me but it seems that this is what's causing the issue. After refreshing the page several times this seems to be what's causing it. I know that the "Image:" syntax isn't standard, but this was being caused by users not following standard wiki syntax. On the wiki affected I'm going to remove all instances of this for the sake of consistency more than anything, however this was slightly puzzling. It's not hugely important, but hopefully this will be fixed to save issues in future.
This works fine for me. Image is an alias for File, and should be perfectly acceptable to use anywhere File is used. Are you sure that the special page wasn't just cached (if MiserMode is enabled)? (if it was it'd have a warning on top saying something like "The following data is cached, and was last updated 23:20, 6 January 2012")

Miser mode is enabled on the wiki, but the special pages update at 12 hour intervals, so that's not the problem. There's still the unresolved issue of why the page is listing used files. I'll look into this more.
