Steps to reproduce:
1. Create an SVG file where the closing </svg> is missing.
2. Select the file on Special:UploadWizard and click "Upload".
A warning is shown: "This file might be corrupt" (which is correct)
3. Click "Retry failed uploads".
4. Repeat step 3 as often as you want, then add the missing </svg> and repeat step 3 once more.
The warning goes away, a preview is shown (which is correct)
5. Click "Continue"
The "Release rights" page is shown, but now there are multiple identical previews of the file, one for each try.

This doesn't seem to happen to me anymore. Now, two things might be happening. First, I couldn't reproduce with the SVG file, because my setup wouldn't properly reproduce the error you got with it. However, the start-immediately patch might have properly dealt with this problem in the course of making it work. If that's the case, then this is fixed. If you can reproduce it now, I'd love to know what your LocalSettings.php is so I can reproduce.

This occurred on Commons, but I can't reproduce it any more. I opened bug 37341 instead.
