The error is:
 exception 'InvalidResponseException' with message 'Invalid response (404): Container has no Objects.' in /usr/local/apache/common-local/php-1.18/extensions/SwiftCloudFiles/php-cloudfiles-1.7.10/cloudfiles.php:1426.

The trace gives out:

 swift.php(37): CF_Container->list_objects(0, NULL, '8/85/Union_Club...')
 [internal function]: wmfPurgeBackendThumbCache( Object(LocalFile), false )

The full filename was Union_Club_Hotel_in_2004.jpg 

User did not get any error on retry and the file is indeed accessible at http://commons.wikimedia.org/wiki/File:Union_Club_Hotel_in_2004.jpg

I have added a wfDebugLog() call on production (local revision number is 2973 ).

I'm guessing this has something to do with the php not liking it when a container is actually empty - wikipedia-commons-local-public.85 has no objects.
