On Mediawiki.org, I'm occasionally seeing two search suggestions boxes at the same time when typing into the search field. I can't figure out how to reproduce this. One box has class="suggestions", the other has id="searchInputSuggest" and class="os-suggest".

I have not seen this anytime recently, but it wasn't very consistent before. I use Vector (assuming by "theme", you mean "skin"...).
