Tested in Semantic Forms 2.4 and 2.4.2. When uploading files, this notice is produced after the upload completes:

Notice: Undefined variable: output in /home/coin/public_html/w/extensions/SemanticForms/specials/SF_UploadWindow.php on line 391

I attached 3 videos of slight variations of the bug, that I made to catch the notice message before the upload popup disappeared.

The bug does not obstruct successful usage of Semantic Forms in any way that I was able to test. The only time the user even gets sufficient time to read the notice message is when they try uploading before SF JavaScript loads completely, as discussed here:

http://www.mediawiki.org/wiki/Extension_talk:Semantic_Forms#Upload_not_inserting_images_into_forms

Even then, the cause of that issue is probably unrelated to this bug.
