Consider the QR Code article, http://en.wikipedia.org/wiki/QR_code. Look at the "Decoding example" section, about 2/3 down the page. I am the author of those 5 images. I updated the images a few days ago. All the updates were successful. But not all 5 updates are visible from the "front page". Instead, you have to drill down, by clicking on (some of) the images, to get to the updated images. The easiest way to see this is to look at the "Q" headline on the right side of the images. I changed fonts in the updates, so the "Q" looks different. Obviously, the updates images should show up on the "front page", as well as in the drill-down close-ups.
When I submitted this bug, images 2, 3 were working correctly, but images 1, 4, 5 weren't. I checked again this morning and now 1 is working correctly, but 4, 5 still aren't. Weird.
And today, #4 is working, but #5 still hasn't propagated. Weirder.
Finally, today, image #5 is propagated. But the question remains: Why did it take MORE THAN 2 WEEKS for updated images to the QR Code article to propagate???
As this bug has not been confirmed, I can only guess that it was due to some cache problems. In the future, you can make MediaWiki purge the existing thumbnails; see <https://commons.wikimedia.org/wiki/Help:Purge> for a manual. I am closing this bug as RESOLVED WORKSFORME, as it seems that the bug does not exist anymore. -- Tomasz (odder)
