Article feedback tool 'improve this page' not fully loaded

When Obama, Golden-Crowned Sparrow and Dre Barrymore home pages are loaded, the pop up (bottom right corner)  'Improve this page' is not fully loaded. As a result the tool tip how to remove the article feedback tool cannot be displayed by clicking the X sign top right of the button that is missing.

'Improve this page' article feedback button was missing for http://en.wikipedia.org/wiki/Barack_Obama?aftv5_form=1&aftv5_link=E in chrome for Win XP.

The feedback form links have since long been removed; this report (although it was perfectly valid at the time) no longer applies
