When hovering mouse over the feedback box, no 'name' was found using Inspect object tool from Microsoft available through windows 7 location "C:\Program Files\Microsoft SDKs\Windows\v7.1\Bin\x64\Inspect.exe" . It was also checked using NVDA and did not pick any prompt for input.

Browser used: FF12

Both the feedback link (removed) and the feedback form (6 instead of 1) have changed since the original report. I don't really know what exactly it references, so I'm pretty confident this no longer applies.
