I can confirm that /mnt/upload6/wikipedia/commons/9/91/Congregation_B'nai_Israel_Synagogue.jpg does not exist on the original-media store. Sounds more like a problem with the upload.

Yeah, probably the move from stash to imagerepo failed. It would be nice if we could figure out why exactly, and we need to make sure that if this happens in the future the entire publication is rolled back properly.
I'm all for doing image and publication in the same transaction, but see Bug 15430.
