Wiki has InstantCommons enabled.

Page embeds [[File:Example.svg|123px|thumbnail|foo]].
Page hasn't been changed in several days.

When viewing the page now it outputs the following before <!DOCTYPE html>:

Those warnings are trapped and sent to wfDebugLog, so this can't happen anymore
