I am getting a strange behavior in Chrome Incognito mode, which doesn't display the feedback form at all on this page:
http://en.wikipedia.org/wiki/Golden-crowned_Sparrow

Dario can reproduce this on his Mac laptop as well. However, it is possible on both machines to access this URL to the CTA 1 in Google Incognito mode:
http://en.wikipedia.org/wiki/Golden-crowned_Sparrow?aftv5_cta=1

Can you reproduce this on your end? Any idea what might be causing this? Is the lack of any cookie disabling AFT5 somehow? or is this an isolated issue with Chrome Incognito?

I can not reproduce this - I think it was more of an artifact caused by chrome's aggressive caching combined with multiple changing parts when deploying (and multiple things needing cache-refresh), rather than a real "issue"

Does it still occur for you?
