In Internet Explorer 9 the feedback box is missing, but the feedback links in the toolbox are present.

That's a rather weird issue.

https://gerrit.wikimedia.org/r/#/c/49067/ should fix this.
The fix has been pushed to prototype, could you please confirm is it works now?
