When I link to a commons image that doesn't exist or has been deleted, it links to File:File:Nyan_cat_250px_frame.PNG

(note the "File:" twice)

What is "it"? Some part of one of our UIs I'm guessing? Or is it a backend issue?

no idea where the cause of the problem is, but it = "www.wikidata.org"

Fixed by the implementation of validators some time ago. See also bug #46030.
