I can't open the activity log after adding note. Sometimes this happens only after moderating and adding a note to 2 feedbacks.

I can reproduce this today (2013-04-18) in the German Wikipedia. I added a note. When I try to click the "show log" link it does not work. Instead a # is added to the URL. I don't see a JavaScript error. It's like the onclick handler is lost and the browser tries to call the href="#" instead.

Indeed; this has not yet been deployed; it's since been merged though - hoping to get it out later today, otherwise next Tuesday
