Users should know which gadgets are active for anons and for other users who never touched their "preferences". 
Also, users should know what will be the result of pressing "Restore default settings".

It is possible to know this by reading Mediawiki:gadgets-definition, but this is not good enough.

Design has no strong stance on this issue, please request a design review prior to integrating any changes in order to maintain a consistent user experience for everyone

i'm not sure if this is directed at me (as the ticket's opener) or not.
if there is anything i can do to help advance it i would. 
what does it mean "please request a design review"? 
should i request something from someone? if so, what exactly should i request? from whom should i request it?

Kipod,

My comment was for anyone who intends on doing development work on this enhancement. Basically this is not a priority for the design group, so we will not be devoting time to this issue, however if a community member would like to devote time to this enhancement, the Design group will provide critique/review support prior to integration of the code.
