Stashing here for future reference so I don't forget.

When uploading (via Special:Upload) an audio file, the ajax "This file already exists" warning produces a thumbnail. The thumbnail is the audio player, but only 2px wide, where really it should be much wider.

Nevermind, this was do to local changes I had going.
