In the diff above, I did the following:
1. Opened the revision on the left using VE
2. Pressed CTRL+END
3. Typed "END"
4. Saved the edit.

For some reason appeared a new image. While investigating this, I also noticed another problem:

Likely caused by an old issue in mis-fired API image calls, fixed ages ago.
