At Commons, we have an Abuse Filter Feedback page: https://commons.wikimedia.org/w/index.php?title=Commons_talk:Abuse_filter

A few months ago, we had a sudden increase in reports by users (mostly anonymous) with no Abuse Filter log entry. Is is possible that not all actions are logged?

I am administrator at Commons and should be able to view the abuse log (except oversighted entries).

This almost certainly isn't a bug, but a seldom concatenation of circumstances:

The user tried several uploads but all of them failed *after* they ran through the AbuseFilter (eg. because of an invalid title or blocked by another extension like the TitleBlacklist). That way the throttle condition was met at some point and the upload attempts were logged as AbuseFilter hits but still the uploads didn't succeed as something else kept blocking them.

Evidence for this thesis is that even the uploads that made it into the AbuseFilter log didn't succeed, so many more could have happened without appearing in any logs.

Sorry, I am a bit forgetful; I created https://commons.wikimedia.org/wiki/Template:Anon-warning-no-edit which most likely was the cause for the increase of anon-users without a filter log.
