This user seems to receive every translation notice twice: https://meta.wikimedia.org/w/index.php?title=User_talk:Akhilan&action=history (note that for the last notification about Admin activity review the message was sent twice, cf. bug 42715 comment 33, so the user got it 4 times).

The only reason why this would logically happen I can think of, is by configuring a message to be delivered to "Talk page" as well as to "Talk page on other wiki","meta.wikimedia.org", as in the attached screenshot. If this indeed is the case, I think "this wiki" should be removed from the drop down list, but that should probably be a hard coded string in the extension configuration. In any case, we will look into it. I hope this is it. The issue of "duplicate deliveries" looks like an administrator issuing the command to notify twice, at least from the logs at https://meta.wikimedia.org/w/index.php?title=Special%3ALog&type=notifytranslators&uselang=en
