CentralAuth is a pain to set up IIRC. It would be nice to have a Vagrant role for it. Ori has suggested making Vagrant provision multiple machines for this, but CentralAuth can run quite happily on one so we should start with one.
Needs some config like this: http://pastebin.com/QLKcEws6 And it also needs a database, which isn't set up by update.php (Technically I think it could go in one of the MediaWiki DBs but generally it has a separate DB.) We'll probably want to run migratePass0.php and migratePass1.php to global-ise any existing users and give the option to specify a user to be added to global_group_permissions with steward rights. From the extension page: "Then, run migrateStewards.php to promote local stewards to global steward status."
