This is a strange edit, where a new editor inserted an image in the image description of an existing image. A bug or something that should be able to be done?

I can't think where off the top of my head, but I have seen small inline images in image descriptions at en.wp before. I think it was being used as a key for the main image or something like that.

It's not common, but its the sort of thing that could be useful on occasion.

Yeah, this is valid (if odd) wikitext, and so VisualEditor supports it.
