Nologo dialog design

Erik/RobLa asked that we work with Jared and the UX team to improve the look of /authorize.

Since the attached design was delivered, we clarified that:
* The dialog will be over a blank special page (since authorizations don't originate on with wiki)
* E3 has already defined styles that we will use (Brandon may have details?), instead of specifying colors and fonts specifically
* We will add the Wiki the authorization applies to (per discussion and wording 9/6)

All of the features requested by Jared have been completed using the E3 styles
