Commons has a standard template that uploads are recommended to use, https://commons.wikimedia.org/wiki/Template:Information . The text of the file page should be changed to use this. To have a good description, you will need to ask the language and the word they're pronouncing, so you should add an initial version of that. This includes an appropriate license. I recommend you use https://commons.wikimedia.org/wiki/Template:Cc-by-sa-3.0 , which is the default one used by UploadWizard if it's your own work. You should show the same message UploadWizard shows to make it clear that you're licensing the recording under that license.
You should probably also add a {{PronunciationRecording}} template for tracking. I don't think it needs any parameters.
You should also use the format and category they want at https://en.wiktionary.org/wiki/Help:Audio_pronunciations#Procedure (where it says, "In a Commons tab...").
