Screenshot of the error

Steps to reproduce:

1.Click on the edit button
2.Add several references in the page
3.Add a template reflist onto it.
4.Insert an image
5.Go to the Media Settings for that image and add a reference and also add the template reflist to the image
6.Save the page

Observed Result:

The reflist that was created for the page disappears and the error appears "Cite error: There are <ref> tags on this page, but the references will not show without a {{reflist}} template (see the help page)"

Test Environment: production
Browser: Chrome Version 26.0.1410.65 & FF 25
OS: MAC OS X 10. 8. 5

Comment 2 Rummana Yasmeen 2013-11-20 02:38:51 UTC
Some more issues with this action:

1. After adding a reflist to the image the whole order of the references in that page gets messed up.

The reference to the image becomes reference with footnote [1] and in the page the order of the reference got changed from [1],[2],[3] to [2],[3],[4].
and the reference of the image gets modified to the [1] reference of the document.

2. Now if you switch to edit source, you will see since the reference in the image is taking the value of first reference of the document as a repetitive reference, the [1] reference of the document gets assigned with ref name= ":1"

But still there is another on the page:Cite error: The named reference :1 was invoked but never defined (see the help page).

(Cool, I never used that template more than once on a page. Now I know it can be done :) https://en.wikipedia.org/wiki/Template:Reflist#Multiple_uses )
