When I did https://www.wikidata.org/w/index.php?title=Wikidata:Requests_for_deletions&diff=89739843&oldid=89732333 using section=new I saw an edit conflict. In this conflict, "original version" is what I want and "my version" is with my text added twice. I guess MediaWiki saw an error (time out?) for the first save, then tried again from the result of first save (but with conflict check base set to the previous revision before my edit), causing a conflict (with the first successful save).

Yes, it is. I believe they have the same root cause.
