When I edit a page, and add a reference to the text and a different reference to an image caption, all in one edit, then the reference text for the image caption is not saved but gets changed to the reference text from the text (this already happens in the preview). You now get two references with the same text, even though you gave them a different text. Example: [https://www.mediawiki.org/w/index.php?title=User%3AFram%2FSandbox&diff=840234&oldid=840227] (the image caption ref had a different text on typing)

I can reproduce this issue.

The key bug here is bug 47344, about using sub-documents instead of the current hack. This would affect media captions and references, and possibly other things.
