Whenever I give user rights and submit the form on the wiki, I see the user rights all unchecked. The support desk has been no help and I am surprised at the ignorance of the community for such an important issue.

This is a wiki farm configuration. Issue has been present since MediaWiki 1.21. Uses GlobalSettings.php available in the same server as other wikis, but not accessible in any way via the web.

Are the permissions assigned? Is the only problem the checkboxes?

According to the database, the person has user rights, it's just there are no checkboxes and that the person isn't really an admin; he does not have access to admin or bureaucrat.

Do you have any extensions installed that might change the way user rights work? If you add groups to an account via the DB, does it work as expected? Are you changing permissions on a different wiki, which may mean that the checkboxes are broken (bug 12518)?
