When I go to watchlist a topic, I often have a hard time getting the cursor off the actionmenu. 
The link for the actionmenu appears to extend right up to the middle of the watchliststar. If I move the mousepointer 1 pixel down from (1st screenshot), then the actionmenu open.

Maybe also consider enlarging it, since this is too small for mobile as well (we currently increase the tap area for a mobile browser)
