Two problems with this (on referata.com):

When I use Special:CreateTemplate, and then show the form associated with the template for a user to enter data to create a page, the fields are in the wrong order, or mislabeled.

Specifically, the name of the second field in the form is that of the last field I entered.  I don't know if "Address Line 1" or "Address1" is a reserved word that is causing a problem ?

When I edit the Template, and the try again to see if it changes the Form, there is a long delay, or it doesn't reflect the changes until I Create the Form again.  In other words, the changes to the Template are not reflected in the associated Form.

I changed "Address1" to "Addrs1".  It didn't fix it.

I also tried removing the last field, so the second to the last field became the last field.  Now this field is the one that shows as the label to the second field.

What fixed it was to edit the Form, not the Template.  I'm not sure, though, how it happened, but it could have been my mistake.

This continues to happen.  If it's my mistake, I haven't figured it out.  I think it's a bug.  I don't know if it's fixed in versions more recent than what referata.com has.
