This looks like it'd require us to implement either subgroups of some kind, or a way to explicitly order the groups (to be able to insert separators at arbitrary positions). If we also want sub-headings as shown in the mockup, we probably actually want the subgroup approach. Do we?

Do we? I was thinking sub-groups, yes (hence the title ;-)). I don't think we'd ever actually use headings of sub-groups in practice, but others might.
