Right now the log entry that gets added to Special:Log/gblrename has no comment, we should add a reason field when the user accepts a rename request, which is then added to the log entry.

A separate "reason" field is useful to add a short description of the action for the log like in Special:GlobalRenameUser. "Notes" instead should be maintained for the complete and exhaustive exposition of the rename reasons.
