To reproduce the error:
1) Use Firefox
2) Upload any new cohort
3) Add a tag to the cohort
4) Click on the cohorts tab (left menu) various times

It seems there's some lost event listener
that catches every click after adding a tag
and adds another tag.
Also it eclipses the refresh button functionality, I think.
