I can't repro.

Are you using an OS/file system that allows forward slashes in filenames? I can't create a file like that.

If the uploaded file doesn't have a forward slash in its original name and you're only talking about entering that as the title in UW, then when I try to do that with the provided example I get "This title contains some undesirable characters. Please remove them." as a validation error.

Same for me, I also cannot reproduce this. Perhaps a broken user script that is interfering...

Or something not related to the username, and an 'incidental' upload error.
