If on ee-flow labs instance I filter RecentChanges by User_talk namespace, the first few changes don't have an <h4> date heading and aren't in a <ul> tag, so they aren't indented.
This only seems to happen for the main and User_talk namespaces where we have Flow boards, so I think it's related to Flow.  Or it could be they have an unexpected timestamp and the page processing doesn't group them.

The WMF core features team tracks this bug on Mingle card https://mingle.corp.wikimedia.org/projects/flow/cards/426, but people from the community are welcome to contribute here and in Gerrit.
