Effect similar to the infamous bug 37073 and bug 54392, but different manifestation. No browser provided, but we need to be proactive because affected users are typically unlikely to look for us (especially if it's Internet Explorer again).

Might be the same reason as bug 65030

This would welcome some browser console output, browser information, file size, and file type, because it might be the same as bug 60247.
Did not manage to reproduce on http://commons.wikimedia.beta.wmflabs.org/wiki/Special:UploadWizard with an SVG file and Chrome 36.

If there is a problem or an error with JavaScript it should be printed in that console:
